Please note TWO separate payments are required. Forms of payment accepted:
- for U.S. Department of State Passport Application Fees: Check or Money Order only (see below for fee schedule);
- for City of Thousand Oaks Acceptance Fees: Cash, Money Order, Checks (payable to CITY OF THOUSAND OAKS), Visa or MasterCard, $35.00
- ALL FEES ARE NON-REFUNDABLE

This facility does not process walk-in applications.
AN APPOINTMENT IS REQUIRED. No exceptions.

2. Passport Acceptance Fees are payable to the "CITY OF THOUSAND OAKS"
$35.00 per passport application (Form DS-11)
The Thousand Oaks Library provides Passport Services by appointment only.
Routine Processing time is currently 7 - 10 weeks.
Expedited Processing time is currently 3 - 5 weeks.
Emergency Passports: If you are traveling within 2 weeks and require an emergency passport you must make an appointment with the National Passport Facility in Los Angeles. Proof of Travel will be required (e.g. airline tickets) Contact (877) 487-2778 to make an appointment.
Our facility can provide the following services by appointment:
- First-time passports. Use form DS-11
- Renewal – Only passports that were issued more than 15 years ago. Use form DS-11
- Passports for Minors – special requirements for minors under the age of 18:

Replacement of lost or stolen passports use form DS-64 and DS-11
ADULT RENEWAL: You can save money by using form DS-82 to renew by mail if your passport was issued less than 15 years ago. If you prefer that we process your request, please use form DS-11 and make an appointment with us. We will treat your request as a “new” passport and you will pay the Acceptance Fee of $35.00 in addition to the fees that you will pay to the U.S. Department of State. Standard processing time is 7 - 10 weeks.
REMINDER: You must provide a Social Security Number on the passport application.
If you fail to provide the information, you will encounter a delay in processing and/or denial of your passport application. You will also be subject to a $500 penalty enforced by the IRS pursuant to Section 6039E of the Internal Revenue Code (26 U.S.C. 6039E). All questions on this matter should be directed to the nearest IRS office. (per US Department of State)
